Find A Dentist
There are multiple ways to find a dentist. Please review all of the options available to determine the best one for you.
Option 1: Search by your Group ID
Where can I find my Group ID?
This number can typically be found in communications sent regarding your employer plan, on dental ID cards, or within our Benefit Tools mobile app.
Option 2: Log in to Online Advantage
Use your Online Advantage for Members login information to identify your network.

Option 3: Select your network below
Don't know your network name? You can find your network name on your dental ID card. If you do not have a dental ID card available, you can access it on Online Advantage.
PPO (Prefered Provider Network) Plan?
Select your PPO Network from the list below.
- If your dental ID card lists DHA or DHA Premier, please select the Assurant® Dental Network.
- If the company you work for is based in North Carolina, please select the Assurant Focus Dental Network®.
- If you do not see your PPO network name in the drop-down, click here.
DHMO or Prepaid Dental Plan?
Select your state and plan below for a listing of available dentists in your area.
Option 4: Generic Search
If you are not a member yet, you can search here to view dentists in our Sun Life Dental Network®. Depending on the network chosen for your plan, the dentists listed may vary.
Option 5: Call Us
If you are unable to find a dentist through our self-service options, please call 1-800-442-7742 for help.
NOTE: It is possible that a provider may have left or joined the network since the publishing of the directory. If the directory information appears to be inaccurate, please contact us by phone at 1-800-434-2638 by email at email@example.com, or by clicking here to let us know.
You can get an interpreter at no cost to talk to your doctor or health plan. To get an interpreter contact the Plan at 1-800-443-2995. Someone who speaks your language can help you.
